New York Medicaid Members with social and health needs may qualify for support to access services like food, housing, and transportation. Local groups called Social Care Networks (SCNs) are available to connect Medicaid Members to services in their community. Learn more.

Social Care Networks (SCNs) are robust networks of Community-Based Organizations (CBOs) and other organizations providing Health Related Social Needs (HRSN) services and coordinating among one another to meet the non-clinical needs of Medicaid members, while also linking with health systems, primary care providers, behavioral health providers, government entities, and other care partners. There are nine regional SCNs across New York, and HEALI is the SCN for Long Island (Nassau and Suffolk counties).

Unite Us is the SCN IT platform selected by HEALI and is the system within which Medicaid members are screened and referred by network organizations. It is how services provided by organizations are tracked for reimbursement. There is no need for Medicaid members to join Unite Us.
