The Social Care Network program brings new or expanded opportunities for providers of screening and navigation services, nutrition, housing, transportation, and enhanced care management services. HEALI connects Health Related Social Needs (HRSN) provider organizations and other partners to support equitable health and life outcomes for our neighbors. We bring together social service and healthcare providers from across Nassau and Suffolk counties to screen Medicaid members, navigate them to HRSN services, and provide necessary services to support the whole person.

Social Care Networks (SCNs) are robust networks of Community-Based Organizations (CBOs) and other organizations providing Health Related Social Needs (HRSN) services and coordinating among one another to meet the non-clinical needs of Medicaid members, while also linking with health systems, primary care providers, behavioral health providers, government entities, and other care partners. There are nine regional SCNs across New York, and HEALI is the SCN for Long Island (Nassau and Suffolk counties).

HEALI’s members are Community-Based Organizations (CBOs), healthcare providers, health-related service providers, and HWCLI, and the network is governed by an elected and independent governing body. Members of the network agree to screen Medicaid members, navigate them to services, and/or provide HRSN services. Members of the SCN sign a membership agreement and will also sign a provider contract to provide reimbursable services through the network.

Unite Us is the SCN IT platform selected by HEALI and is the system within which Medicaid members are screened and referred by network organizations. It is how services provided by organizations are tracked for reimbursement. There is no need for Medicaid members to join Unite Us.

HEALI was founded as a coalition within the Health and Welfare Council of Long Island (HWCLI) with support from the New York State Department of Health as part of the Delivery System Reform Incentive Payment (DSRIP), a program that uses federal funds to help improve healthcare delivery systems. In 2024, HEALI was named Long Island’s SCN, with HWCLI named as lead entity.
